Out-of-State Applicants will need to follow these simple instructions

Download and print out the forms in the packet above. Detailed instructions for filling out the Out-of-State Fingerprint Application forms are included in the forms packet. Here is an overview of the application steps:

Print out two copies of the standard FBI FD-258 fingerprint card form. It is acceptable to print the FBI FD-258 form on standard white printer/copy paper.

International Applicant Note: International applicants must also use the standard FBI FD-258 fingerprint card form; do not submit fingerprints on non-FBI fingerprint card forms.
Have two sets of fingerprints taken and placed on the FBI FD-258 card or printed form, using the ink and roll process. “Reason fingerprinted” on the card should specify which license (nursing, PERC, etc…). For questions, please call 866-361-9944 or send us an email at [email protected].

Note: We will also accept fingerprint cards created by printing on to the cards from an electronic capture by an authorized fingerprint provider such as a local law enforcement agency or licensed fingerprint contractor.
The applicant only fills out Section 1 of the Identity Verification Certifying Statement (OOS-FP). Section 2 is filled out by the law enforcement or fingerprinting agent at the time of fingerprinting. The TCN field in this section will remain blank (to be completed by the livescan vendor). Section 3 is completed by the Accurate Biometrics technician.
Fill out the Out-of-State Fingerprinting/Consent Form. You must include your phone number or email address in the event we need to contact you regarding your fingerprint card. Please sign the form to acknowledge that you have read and agree to the data privacy and biometric information policy.
The standard client fee is $67, please complete the payment form included in the download packet. Clients with a customer service agreement, please submit the form supplied by our Customer Support Team with pricing details and your payment or contact us for further information.

For Status On Your Illinois State Police Fingerprint Background Check

For all applicants under the Illinois Department of Financial and Professional Regulation: If you do not receive a fingerprint receipt from Accurate Biometrics via email within 2 weeks of mailing your completed fingerprint processing packet, please call our customer service department toll free at 866-361-9944 or send us an email at [email protected].

If we verify that your prints have been processed and your Requesting Agency has not received a response from the Illinois State Police (ISP), you will need to contact the ISP for an update. You must have the Transaction Control Number from your receipt

The ISP prefers an email request at [email protected]; to contact the ISP by phone call 815-740-5160, then select option #2.

Important Notes

We will process your fingerprint card, convert it to a digital format, and send it electronically to the Illinois State Police and FBI for the background check. The results will be forwarded directly from the ISP to the designated Illinois Requestor Agency.
Do not send any part of your licensing application to Accurate Biometrics. Licensing applications must be sent to the Requesting Agency that issues the license.
Fingerprints are submitted real-time on the day the fingerprints by mail are received. Once fingerprints have been submitted NO REFUND REQUESTS CAN BE GRANTED.
Your fingerprint card and accompanying forms will be shredded after we have confirmed successful processing by the ISP and receipt of the results by your Requesting Agency.

Out-of-state fingerprint card processing for Florida.

Florida regulation mandates out-of-state applicants must submit electronic fingerprints with the appropriate State of Florida ORI numbers provided by the State agency requesting the background check. Applicants can utilize standard FBI fingerprint cards (FD-258) or FBI FD-258 form.

Level 2 Background Check Requirements

Florida regulation mandates out-of-state applicants must submit electronic fingerprints with the appropriate State of Florida ORI numbers provided by the State agency requesting the background check.

Applicants can utilize standard FBI fingerprint cards (FD-258) or FBI FD-258 form.

By-Mail Fingerprint Ink Card Scanning and Processing

Applicants will need to follow these simple instructions:

Please download the Forms Packet above. You can use an FBI FD-258 fingerprint card from your local sheriff's office, police department or the State agency that is requesting your fingerprint check, or you can use the one provided in the Forms Packet.

Note: the FBI FD-258 fingerprint form may be printed on letter sized paper.

Note: All data fields highlighted in yellow must be written/printed legibly and completely filled-in on the fingerprint card or form to expedite the fingerprint card scan service. Be sure to include the ORI Number for your fingerprinting purpose.
Take the APPLICANT Fingerprint card or form to your local Police Department or Sheriff’s Office to request “Rolled Fingerprints”. (Fill out the APPLICANT Fingerprint card with BLACK INK ONLY in the sections of the card shown highlighted in yellow in the example included in the pdf).
Payment can be made in the form of a money order or cashier’s check made payable to Accurate Biometrics, or with the completed Credit Card Payment Form included in the Forms Packet.
Please review the data privacy information in the Forms Packet, and fill in and sign the Fingerprint Cardscan Applicant Consent form.
You need return only the 2 original copies of the FBI FD-258 fingerprint card form, the Credit Card Payment Form (or other accepted payment), and the Fingerprint Cardscan Applicant Consent Form.

Note: Our card scan service cannot be provided without including an enclosed payment and the signed Fingerprint Cardscan Applicant Consent form. Not having all the required forms and payment will delay processing.

Additional Information

Accurate Biometrics will convert your fingerprint card or form into live scan and transmit to the Florida Department of Law Enforcement for processing.

The State Agency requesting your Level 2 background check will receive your Florida and FBI criminal history record. This will complete your Level 2 Background Check steps.

Accurate Biometrics will email you a Transaction Control Number (TCN) Report that verifies your live scan submission has been processed. This will also serve as your receipt.

VERY IMPORTANT: We cannot process your fingerprint card without an ORI number. (Check with the State agency requesting the background check to confirm your ORI number.)

Mail-In Applications for the Department of Health

Florida Department of Health (e.g. Board of Nursing, Board of Medicine) applicants by-mail can get a background check for licensing or endorsement purposes ONLY. AHCA Applications

AHCA applicants must have their prints captured electronically and can do so at one of our Fingerprint Collection Network Kiosks. A photo will be taken at the kiosk, which satisfies both the photo and the fingerprinting requirements. To find a Kiosk location near you click here.
