Instructions for Personal Review of FBI History

This service is available for US Citizens and US Permanent Residents only.

Hello SeoulESL teachers,

SeoulESL has partnered with Accurate Biometrics to provide SeoulESL teachers with the best FBI channeling service for the lowest national rate and a speedy turnaround time.

Accurate Biometrics and SeoulESL have created a customized process specifically for the needs of teachers applying to teach English in South Korea. You’ll have the option of receiving a physical copy of the FBI check on White Secure Watermark paper to ensure that it can be apostilled by the US Department of State and accepted by Korean immigration.

Please follow the steps below to secure the correct FBI criminal history record and have it mailed directly to you.

Follow these simple steps to get your FBI Background Check:

Step 1: Complete the Applicant Information Form

  • Print and complete the Applicant Information Form.
  • Refer to the sample form to ensure you have completed all the required data fields accurately.
  • Include your complete mailing address in the USA. Please also provide your telephone number and email address.

Step 2: Get Fingerprinted

  • Print two (2) standard fingerprint forms (FD258) on regular printer paper from the following link: Download the FBI 258 card
  • Complete the fields highlighted in yellow on the top section of BOTH fingerprint forms. You must follow the detailed instructions on the sample to ensure the fingerprint form is completed correctly.
  • Contact your local law enforcement agency (local police, sheriff’s department) to make arrangements to have your hard copy ink fingerprints taken directly on each of the two fingerprint forms. Photocopies of your fingerprints are not permitted.
  • Individuals may NOT fingerprint themselves; fingerprints MUST be obtained from a law enforcement agency or through Accurate Biometrics, pursuant to the Departmental Order Requirements.
  • Each fingerprint form FD258 must include rolled impressions of all 10 fingerprints and impressions of all 10 fingerprints taken simultaneously (these are sometimes referred to as plain or flat impressions).

Step 3: Select Shipping Option 2 or 3

FBI response must be returned to the applicant’s address or applicant’s attorney. The response sent by Accurate Biometrics, will be printed on secure paper. You can select the following options of shipping:

Step 4: Submit Payment.

Option 1: Obtain a money order or cashier’s check for the fee based on desired service in U.S. dollars made payable to Accurate Biometrics. Please be sure to sign where required.

Option 2: Pay by Credit card using the Credit Card Payment Form. Don’t forget to include the expiration date and three digit security code of the credit card that your are using:


  • Cash, or personal checks WILL NOT be accepted for mail in service.
  • Payment must be for the exact amount.

Step 5: Mail the required items.

Mail your completed packet with all four items to the following address:

  2. One original, complete Fingerprint form: FD258 (see step 2)
  3. Second, original, complete Fingerprint form: FD258 (see step 2)

Fingerprints will be scanned and sent to the FBI the same day cards are received by Accurate Biometrics.

The FBI result/response is generated by Accurate Biometrics in accordance with the FBI issued “Channeler Requirements for the Departmental Order Process: dated June 10, 2011. Accurate Biometrics is required to print the criminal history response exactly as it is electronically received from the CJIS Division of the FBI.
