Instructions for obtaining your FBI Identity History Summary Check (IHSC)

This service is available for US Citizens and US Permanent Residents only.

NOTE: Per FBI requirements, applications submitted from outside the U.S. cannot be accepted or processed. Results can only be picked up within the United States or its territories. International applications (outside the U.S.) need to be made directly to the FBI.

Accurate Biometrics and Korvia have created a customized process specifically for the needs of teachers applying to teach English in South Korea. Participants will only have the option of receiving a physical copy of the IHSC on white secure watermarked paper.

Please follow the steps below to correctly apply for your FBI Identity History Summary Check and have it mailed directly to you.

To get fingerprinting for your FBI national background check, follow the simple steps below

OPTION 1: Mail-In Application

Step 1: Complete the Applicant Information Form

Print and complete the Applicant Information Form.
Refer to the sample form to ensure you have completed all the required data fields accurately.
Include your complete mailing address. Please provide your telephone number and email address.

Step 2: Get Fingerprinted

Print two (2) standard fingerprint forms (FD258) on regular printer paper from the Forms Packet above.
Complete the fields highlighted in yellow on the top section of BOTH fingerprint forms. You must follow the detailed instructions on the sample to ensure the fingerprint form is completed correctly.
Contact your local law enforcement agency (local police, sheriff’s department) to make arrangements to have your hard copy ink fingerprints taken directly on each of the two fingerprint forms. Photocopies of your fingerprints are not permitted.
Individuals may NOT fingerprint themselves; fingerprints MUST be obtained from a law enforcement agency or through Accurate Biometrics, pursuant to the Departmental Order Requirements.
Each fingerprint form FD258 must include rolled impressions of all 10 fingerprints and impressions of all 10 fingerprints taken simultaneously (these are sometimes referred to as plain or flat impressions).

Step 3: Select Your Pricing and Shipping Option

Besides your online results, you can request a hard copy of your results on secure watermarked paper, with either USPS mail delivery or 2-day Priority Service. Due to rising carrier fees and logistics costs, effective July 1, 2026, our rate for UPS 2nd Day Air shipping will be adjusted to $120 per shipment.

The FBI response must be returned to the applicant’s address or applicant’s attorney. The response sent by Accurate Biometrics, will be printed on White Secure Watermark paper (an attorney release letter on attorney’s letterhead must accompany FBI 258 card, signed by the attorney and applicant).

Few vendors provide the option of a hard copy of your results like we do. Only we include delivery tracking with all mailed responses.

NOTE: Additional, white secure watermarked copies of your federal background check are $5 each. The request for additional copies must be submitted on a cover letter indicating the number of additional copies.

Step 4: Submit Payment using one of the following choices

Money Order or Cashier’s Check: Obtain a money order or cashier’s check for the fee based on desired service in U.S. dollars made payable to Accurate Biometrics. Please be sure to sign where required.

Pay by Credit Card: Pay by credit card using the Credit Card Payment Form. Don’t forget to include the expiration date and three digit security code of the credit card that your are using.

Important

Cash or personal checks WILL NOT be accepted for mail in service.
Payment must be for the exact amount.

Step 5: Complete the Fingerprint Cardscan Applicant Consent Form

Please review the Biometrics Retention and Destruction Policy information included in the Forms Packet, then complete and sign the Fingerprint Cardscan Applicant Consent Form.

Step 6: Mail the required items.

Check that your mail packet is complete and then mail it to the address below.

Fingerprints will be scanned and sent to the FBI the same day cards are received by Accurate Biometrics.

The FBI result/response is generated by Accurate Biometrics in accordance with the FBI issued “Channeler Requirements for the Departmental Order Process: dated June 10, 2011. Accurate Biometrics is required to print the criminal history response exactly as it is electronically received from the CJIS Division of the FBI.

Dear Missionary:

Accurate Biometrics and The Church of Jesus Christ of Latter-Day Saints are partnering to provide our missionaries with the best FBI channeling service. Please follow the steps below to obtain your FBI Channeler form(s). By using the links below, you obtain the benefit of the discounted rate the Church has negotiated with Accurate Biometrics.

Once you receive your results from Accurate Biometrics, please follow the instructions in your visa packet. The fingerprint based background check is needed for all missionaries as they prepare to live, work and travel to a foreign country. You have two options for submitting your fingerprints. You can provide your fingerprints electronically by visiting one of the Kiosks in our Nationwide Collection Network (pre-registration is required), or you can have your fingerprints captured on the standard FBI 258-FD card form and mail them in for processing.
