Illinois Video Gaming

The Illinois Gaming Board (IGB) requires State and FBI fingerprint-based background checks to license Owner/Operators for video gaming facilities. Video gaming Terminal Technicians and Terminal Handlers also require fingerprinting.

For more information for applicants to get fingerprinted for Illinois Video Gaming, follow the simple steps below:

In-State:

  1. Fill out Form: Complete a Fingerprint Authorization Form by checking Video Gaming Terminal/Technician option and entering your demographic information. The form is available for download by clicking here and is also available at all Accurate Biometrics fingerprint locations in Illinois (and can be completed on site). Complete the form with your personal information.

  2. Find the closest Accurate Biometrics fingerprint location and its hours of operation by clicking here. No appointments necessary at any of our sites!

  3. What to Bring: Bring the completed form, a valid, government issued, picture ID (such as a driver's license, State ID or passport). A receipt will be provided after fingerprinting that includes the name, date and Transaction Control Number (TCN).

  4. A receipt will be provided after fingerprinting that includes the name, date and Transaction Control Number (TCN). The TCN is used for tracking your transaction. Please retain this receipt that includes your TCN number in case there are any follow up questions regarding your fingerprint submission.

  5. Results will be sent directly to the Illinois Gaming Board.


Out-of-State:

Accurate Biometrics is authorized by the Illinois Gaming Board to process fingerprints for Out of State applicants for Owner/Operator Gaming license. The Gaming Board will send Accurate Biometrics your ink card for processing once your application has been approved.

  1. Fill out Form: Complete a Fingerprint Authorization Form by checking Video Gaming Terminal/Technician option and entering your demographic information. The form is available for download by clicking here and is also available at all Accurate Biometrics fingerprint locations in Illinois (and can be completed on site). Complete the form with your personal information.

  2. Find the closest Accurate Biometrics fingerprint location and its hours of operation by clicking here. No appointments necessary at any of our sites!
