Illinois UCIA Law (20 ILCS 2635/Illinois Uniform Conviction Information Act) allows any individual to
access personal criminal history record conviction information on file with the Illinois State Police
(ISP). The response received will contain all conviction data compiled by the ISP. An individual may
grant permission to send the response to another individual or entity, such as an employer, by listing
them as the third party requester. .

Bring a valid government-issued photo ID to an Accurate Biometrics Fingerprint
location. (Driver’s license, state ID, passport, etc.) and payment if required*.
We will capture your fingerprints and transmit them, the same day, to the Illinois
state Police (ISP). A fingerprint services receipt noting the assigned Transaction
Control Number (TCN) will be provided to the applicant at the completion of the
fingerprint capture process.
Results will be mailed to the requesting individual from the IL State Police and
should be received within 15 days, although it may take longer.
If you do not receive your results in 15 days, you must call the ISP to have them
resend your response. The ISP prefers an email request at [email protected];
to contact the ISP by phone call 815-740-5160, then select option #2.
If the applicant does not contact ISP for results they have not received within 30
days, the applicant may need to resubmit fingerprints. UCIA fingerprint
submission requests expire 30 days after they have been processed by the
ISP.
*Price: The walk-in client fee is $45, due at the time of

Out-of-State Applicants for Illinois UCIA checks, follow the simple steps
below:
Download and print out the forms in the packet above. Detailed instructions for filling
out the Out-of-State Fingerprint Application forms are included in the forms packet.
Here is an overview of the application steps:
Print out two copies of the standard FBI FD-258 fingerprint card form. It is
acceptable to print the FBI FD-258 form on standard white printer/copy paper.
Have two sets of fingerprints taken and placed on the FBI FD-258 card or printed
form, using the ink and roll process. Mark “UCIA” as the reason for fingerprinting.
Note: We will also accept fingerprint cards created by printing on to
the cards from an electronic capture by an authorized fingerprint provider such as a
local law enforcement agency or licensed fingerprint contractor.
Fill out the Fingerprint Authorization Form. You must include your phone number or
email address in the event we need to contact you regarding your fingerprint card.
We require a payment of $45.00 included with the application. Credit card payment is
preferred. We do not accept cash or personal checks. All major credit cards and

The Illinois state Uniform Conviction Information Act (UCIA) fingerprint-based background check provides
an opportunity for individuals or employers to obtain criminal history information on file with the
state of Illinois. Results of this check are mailed directly to the requesting agency or individual by
the Illinois state Police (ISP).
Important Notes:
Illinois UCIA Act allows any company, agency or 3rd party to access personal criminal history record
conviction information on file with the Illinois State Police (ISP). The UCIA is also commonly used
by 3rd parties to receive CHRI information that do not have access to fingerprint-based Criminal
History Record Information (CHRI) information under current Illinois law. Examples of those who use
this check are landlords, those looking for caregivers or household help, pre-employment and social
networking. The response received will contain all conviction data collected and maintained by the
Illinois State Police. An individual may grant permission to send the response to another individual
or entity, such as an employer, by listing the individual party as the requester.
A fingerprint-based background check returns more accurate and complete results than a
non-biometric, name-based check. Fingerprint-based checks eliminate the risk associated with
background checks in which an individual may have a common name, may use an alias, or may indicate a
false date-of-birth – any of which could adversely affect the results of a non-biometric background
check of the ISP’s computerized criminal history record files.
The UCIA check returns only Illinois Conviction Information. For complete record information,
including Open Arrests with or without Disposition Information, an individual can
request an Illinois Access and Review check. Both
UCIA and Access and Review fingerprint-based checks are a “snapshot” in time, and do not provide
updated criminal history information when a record is amended.
