EMS/EMT FINGERPRINT BACKGROUND CHECKS

Emergency Medical Services (EMS) and Emergency Medical Technicians (EMT) licensees are in a position of trust given that they respond to emergency situations in which adults and/or children may be incapacitated due to a sudden illness or accident. EMS/EMT licensees may have access to various prescription medications, including narcotics.

Those interested in pursuing an EMS/EMT career may want to request their own background check to make sure nothing in their criminal history would exclude them from obtaining a license in the future.

Among the EMS careers an individual may want to request a fingerprint-based background check for are:

  • EMT – Basic, Intermediate and Advanced
  • Paramedic
  • Lead Instructor
  • Emergency Medical Dispatcher
  • Emergency Medical Responder

The Illinois State Uniform Conviction Information Act (UCIA) fingerprint-based background check provides an opportunity for individuals or employers to obtain criminal history information on file with the State of Illinois. The response will contain all conviction data compiled and maintained by the Illinois State Police (ISP). An individual may grant permission to send the response to another individual or entity, such as an employer, by listing the individual party as the requestor. Results of this check are mailed directly to the individual or requesting agency by the ISP.

Illinois UCIA checks for individual EMS/EMT applicants, follow the simple steps below:

  1. Download the Fingerprint Authorization Form above and complete the form with your required personal information.

  2. Find a convenient Accurate Biometrics fingerprint location and hours of operation by clicking here. No appointments necessary!

  3. Bring a valid government-issued photo ID to an Accurate Biometrics Fingerprint location. (Driver’s license, State ID, passport, etc.) and payment if required*.

  4. We will capture your fingerprints and transmit them, the same day, to the Illinois State Police (ISP). A fingerprint services receipt noting the assigned Transaction Control Number (TCN) will be provided to the applicant at the completion of the fingerprint capture process.

  5. Results will be mailed to the requesting individual or agency from the IL State Police and should be received within 15 days, although it may take longer.

  6. If you do not receive your results in 15 days, you must call the ISP to have them resend your response. The ISP prefers an email request at [email protected]; to contact the ISP by phone call 815-740-5160, then select option #2.

  7. If the applicant does not contact ISP for results they have not received within 30 days, the applicant may need to resubmit fingerprints. UCIA fingerprint submission requests expire 30 days after they have been processed by the ISP.

Illinois UCIA checks for EMS/EMT schools or companies, follow the simple steps below

  1. Download the Fingerprint Authorization Form above and complete the form with your required personal information.
    • Enter the Requestor information (name of Requestor and correct mailing address to whom the results are to be mailed by the Illinois State Police).

Out-of-State Applicants for Illinois UCIA check for EMS/EMT, follow the simple steps below:

Download Out-of-State Forms Packet

Download and print out the forms in the packet above. Detailed instructions for filling out the Out-of-State Fingerprint Application forms are included in the forms packet. Here is an overview of the application steps:

  1. Print out two copies of the standard FBI FD-258 fingerprint card form. It is acceptable to print the FBI FD-258 form on standard white printer/copy paper.

  2. Have two sets of fingerprints taken and placed on the FBI FD-258 card or printed form, using the ink and roll process. Mark “UCIA” as the reason for fingerprinting.

    Note: We will also accept fingerprint cards created by printing on to the cards from an electronic capture by an authorized fingerprint provider such as a local law enforcement agency or licensed fingerprint contractor.

  3. Fill out the Fingerprint Authorization Form. You must include your phone number or email address in the event we need to contact you regarding your fingerprint card.

  4. We require a payment of $45.00 included with the application. Credit card payment is preferred. We do not accept cash or personal checks. All major credit cards and money orders are accepted. Click here to view the complete list of payment terms.

  5. Check to be sure to include the following items when mailing your fingerprint card for processing:
    • Completed FBI FD-258 format fingerprint card.
    • Completed Out-of-State Fingerprinting Form
    • Completed Identity Verification Certifying Statement (OOS-FP)
    • Payment-Money Order, Company Check or Credit Card Payment Form (NO CASH OR PERSONAL CHECKS)

Note, a felony conviction may not result in an automatic disqualification of an applicant.

Illinois Law 77 Ill Adm. Code 515.190(e) lists the six factors that are taken into account when evaluating whether the applicant’s criminal history may present a risk to patients.

Important Notes:

  • Illinois UCIA Act allows any company, agency or 3rd party to access personal criminal history record conviction information on file with the Illinois State Police (ISP). The UCIA is also commonly used by 3rd parties to receive CHRI information that do not have access to fingerprint-based Criminal History Record Information (CHRI) information under current Illinois law. Examples of those who use this check are landlords, those looking for caregivers or household help, pre-employment and social networking. The response received will contain all conviction data collected and maintained by the Illinois State Police. An individual may grant permission to send the response to another individual or entity, such as an employer, by listing the individual party as the requester.

  • A fingerprint-based background check returns more accurate and complete results than a non-biometric, name-based check. Fingerprint-based checks eliminate the risk associated with background checks in which an individual may have a common name, may use an alias, or may indicate a false date-of-birth – any of which could adversely affect the results of a non-biometric background check of the ISP’s computerized criminal history record files.

  • The UCIA check returns only Illinois Conviction Information. For complete record information, including Open Arrests with or without Disposition Information, an individual can request an Illinois Access and Review check. Both UCIA and Access and Review fingerprint-based checks are a “snapshot” in time, and do not provide updated criminal history information when a record is amended.

  • To find out whether your national criminal history record report is complete, accurate and up-to-date, you can request a personal FBI background check (also known as an Identity History Summary Check report or Departmental Order 566-73).
