Follow these simple steps to complete your DOH Level 2 fingerprint background check requirement:

  1. Go to the Florida Department of Health website to obtain an ORI number and your Electronic Fingerprint Form: http://www.floridahealth.gov/licensing-and-regulation/index.html

  2. Use your Live Scan Request Form as a reference to register online for fingerprinting.

  3. Bring your registration confirmation email (printed or on your phone) along with your current (not expired) driver’s license, government issued identification card, or passport to the fingerprinting Kiosk location of your choosing. (You will choose your location after clicking the GET FINGERPRINTING button below.) Please call us at (866) 361-9944 if you have any questions.

  4. The fingerprints and photo that are captured by the technician at the fingerprinting Kiosk are sent to the Florida Department of Law Enforcement (FDLE). The FDLE then sends the fingerprints to the FBI for processing. The results are then disseminated back to the Florida Department of Health Clearinghouse. Once the results are received from FDLE, the average processing time for clearance is approximately 3-5 business days.

  5. You will receive an email providing you with a Transaction Control Number (TCN) that verifies your live scan submission has been processed. The TCN number can be given to the company that is requesting the background check as it will help them track results. IMPORTANT: Please save your TCN number for reference.

Out-of-State Applicants

Applicants who submit an initial licensure application to the Florida Department of Health (DOH) for a profession that requires a Level 2 Background Screening, must submit fingerprints electronically along with a photo to be in compliance with “The Clearinghouse.”

The Clearinghouse allows the results of the criminal history checks to be shared among specified agencies when a person has applied to be employed, volunteer, be licensed, or enter into a contract that requires a state and national fingerprint-based criminal history check.
