FDLE Approved Live Scan Vendor

Employees at Assisted Living Facilities are required to get a Level 2 fingerprint-based background check before they can start working.

In order to work with a company that is regulated through the Agency for Persons with Disability (APD) and APD CDC Plus, all applicants are required to undergo a Level 2 background check, that includes a live scan fingerprinting process.

The APD & APD CDC Plus uses a Clearinghouse that allows the results of the criminal history checks to be shared with multiple state agencies when an individual is looking to work with, volunteer at, or contract through any organization providing services to children, the elderly and disabled individuals. Once you have your background checked, you can move between agencies (especially those that volunteer for multiples organizations) and not have to get a new fingerprint background check each time.


  1. The company that is requesting your Level 2 background check has to go on the APD/APD CDC Plus web Portal and generate a level 2 background check request.
    After they have registered you into the APD/APD CDC Plus Clearinghouse portal they must provide you with a Live Scan Request Form that their system automatically generates with an ORI number and a Screening ID number.

  2. Bring in your Live Scan Request Form along with your current (not expired) driver’s license, government issued identification card, or passport to Accurate Biometrics at 5005 W. Laurel Street, Ste. 111, Tampa. Please call us at (813) 586-3400 if you have any questions.

  3. The fingerprint and photo are captured by Accurate Biometrics then sent to the Florida Department of Law Enforcement (FDLE). FDLE then sends the fingerprints to the FBI for their nationwide processing. The results are then disseminated back to the Agency for Persons with Disabilities/Department of Children & Families Clearinghouse.

  4. Accurate Biometrics will provide you with a Transaction Control Number (TCN) report that verifies your live scan submission has been processed. The TCN number can be given to the company that is requesting the background check as it will help them track results. Once the results are received from FDLE, the average processing time for clearance is approximately 3-5 business days.
